I am concerned that teachers (math teachers) are not be prepared to make grade level judgements. ( We have spoken in that past about how you do not necessarily have to formally test students), it possible to provide them with measurement tools and/or guidelines? Samia and I have an online testing link that we could share.

I agree with Cathy in terms of receiving more feedback using this form. The only suggestion I have would be a way for teachers to upload an actual copy of student’s work via pdf or jpeg file, which will be more accurate along with the teacher’s description.

I think the form is well done and convenient. But, I would like a follow-up from Dr. Kassoff on his determination of the kid’s condition and some coaching on to handle them. This is especially important in demanding classes, like physics, which tends to put more pressure on the kids and raise their anxiety levels.

This form is comprehensive and user-friendly. Responses will give a complete picture of the student academically and behaviorally in the respective subject classes. You must have put 20 hours minimum into comprising this form. I believe teacher responses will be more detailed and more timely than the paper evalulation form. I appreciate receiving a copy of my online responses in an email.
Perhaps you could add a narrative description section for homework and time management.
Also, change EXECUTIVE FUNTION to FUNCTION.
I believe that these questions are wonderful and will allow you to receive a more whole, complete picture of each student. Looking forward to using this in the future. Quick and simple, love it!
Just got my first chance to “test drive” the form using an actual student. I thought the form was excellent and allowed me to supply meaningful feedback in an efficient manner. The only change I might suggest is adding an option under the “Test Taking Style” section that is neutral — perhaps something like “works appropriately” or “keeps pace with classmates” or something to that effect. Overall the form is great and I think it’s a huge improvement over the old system!!
